A tooth that hurts only when you bite can be confusing because it may feel normal at rest.
That pattern is still important.
The American Association of Endodontists notes that sharp pain when biting may be associated with decay, a loose filling, a crack, or damage to the pulp inside the tooth. AAE: Tooth Pain
The symptom cannot identify the cause by itself, which is why examination matters.
Could the tooth be cracked?
Yes.
Cracked teeth often cause inconsistent pain with chewing, especially when pressure is applied or released.
The AAE explains that cracks can flex during chewing and irritate the pulp. Symptoms may come and go, making the exact tooth difficult to identify. AAE: Cracked Teeth
A crack can range from a shallow enamel line to a deeper fracture that affects the pulp or root.
Our chipped and cracked tooth guide explains why crack location and depth change the treatment plan.
Could a filling be loose or broken?
Yes.
A damaged restoration can shift under pressure or expose sensitive tooth structure.
Pain with biting may also develop if new decay forms around or under an older filling.
If the filling recently changed, see our guide on broken or lost fillings only after it is live. Until then, contact the office directly rather than trying a home repair.
Can the bite itself cause pain?
Sometimes.
After a new filling or crown, a restoration that contacts too heavily can make the tooth feel sore when chewing.
If discomfort began immediately after dental work and the tooth feels as though it “hits first,” call the dentist who completed the treatment.
A simple bite adjustment may be appropriate in some cases, but pain should not automatically be blamed on the bite without checking the tooth.
Could the pulp or root be inflamed?
Yes.
The pulp can become inflamed because of deep decay, cracks, repeated dental work, or trauma.
Pain when biting can also occur when tissues around the root become inflamed.
If the pulp cannot heal, root canal treatment may be needed.
Our guide on whether you may need a root canal explains why dentists combine symptoms with examination and imaging rather than diagnosing from pain alone.
Could an abscess cause pain when biting?
Yes.
An abscessed tooth may cause pain with chewing, swelling, gum tenderness, drainage, or a bad taste.
AAE guidance notes that ongoing symptoms from an infected tooth should be evaluated even if the pain is not constant. AAE: Abscessed Teeth
See our dental abscess guide for warning signs that need prompt attention.
Why might the dentist take an X-ray?
X-rays can show information that is not visible during a visual examination, including bone changes, deep decay, root shape, and some signs of infection.
But an X-ray alone may not show every crack.
The dentist may also use:
- Bite testing
- Temperature testing
- Percussion
- Gum probing
- Visual magnification
- Transillumination
- Comparison with neighboring teeth
The goal is to reproduce and localize the symptom safely.
Should you keep chewing on the tooth to test it?
No.
Repeatedly biting hard objects to “find the crack” can worsen damage.
Until the tooth is evaluated:
- Chew on the other side if needed
- Avoid hard foods
- Keep the area clean
- Do not place aspirin on the tooth or gum
- Call if swelling or pain worsens
When is pain with biting urgent?
Seek prompt dental care when pain is severe, worsening, or associated with:
- Facial or gum swelling
- Fever
- Drainage
- A broken tooth
- A loose tooth
- Spontaneous throbbing pain
- Difficulty opening the mouth
Rapid facial swelling or trouble breathing or swallowing requires urgent medical attention.
