A permanent adult tooth should not normally move when you touch it or chew.

If a tooth suddenly feels loose, the safest approach is to have it evaluated rather than trying to tighten it, splint it, or wait for it to become painful.

One important cause is periodontal disease. NIDCR explains that advanced gum disease can damage the bone and tissues supporting teeth, leading to looseness and shifting. NIDCR: Periodontal Disease

But gum disease is not the only possible explanation.

What can make an adult tooth loose?

Possible causes include:

  • Periodontal disease and supporting-bone loss
  • Dental trauma
  • Infection around a tooth
  • A fracture
  • Excessive bite forces or grinding
  • Changes around a restoration
  • Bone loss from another dental condition

The cause cannot be identified reliably by how the tooth feels alone.

A tooth may be loose without severe pain, and a painful tooth is not necessarily loose.

How does gum disease loosen teeth?

Periodontitis affects the tissues that hold teeth in place.

NIDCR explains that dentists evaluate gum inflammation, pocket depth, tooth mobility, medical history, and bone levels on X-rays when diagnosing gum disease. NIDCR: Periodontal Disease

Other signs may include:

  • Bleeding gums
  • Gum recession
  • Persistent bad breath
  • Pain when chewing
  • Spaces opening between teeth
  • A change in the bite

Our guides on gum pockets and receding gums explain related periodontal findings.

What if the looseness started after an injury?

A hit to the mouth can damage the tooth, root, periodontal ligament, or surrounding bone.

Even if the tooth is still in place, it may need prompt dental evaluation.

Do not repeatedly wiggle the tooth to test it.

If a tooth has been completely knocked out, that is a separate emergency. Our knocked-out tooth guide explains the immediate steps for a permanent tooth.

Can teeth grinding make a tooth feel loose?

Clenching and grinding can place heavy forces on teeth and restorations.

In some patients, bite trauma can contribute to mobility, especially when the supporting tissues are already compromised.

However, a night guard should not be assumed to solve unexplained tooth mobility. The dentist first needs to determine whether gum disease, fracture, infection, or another problem is present.

Our article on night guards for teeth grinding explains why diagnosis comes before appliance treatment.

Could a cracked tooth feel loose?

A fracture can change how a tooth moves or how pressure is transmitted when you bite.

Cracks vary widely in depth and severity. Some affect only enamel; others extend deeper toward the root.

Pain when biting, sensitivity, or a visible crack should be examined. Our chipped and cracked tooth guide explains common evaluation factors.

What will the dentist check?

An examination may include:

  • How much the tooth moves
  • Gum condition and pocket depth
  • Whether there is swelling or drainage
  • Bite contacts
  • The condition of fillings or crowns
  • Sensitivity or pain patterns
  • X-rays to assess roots and supporting bone when indicated

The treatment depends on the cause.

Options can range from periodontal treatment or bite management to restorative treatment, root canal therapy, splinting after trauma, or extraction when a tooth cannot be saved.

Is a loose tooth always an emergency?

Not every loose tooth requires an emergency-room visit, but it should not be ignored.

Seek prompt dental care if the tooth becomes suddenly mobile, especially after trauma or with swelling, drainage, severe pain, or fever.

If facial swelling is rapidly increasing or you have trouble breathing or swallowing, seek urgent medical care.

Can a loose tooth tighten again?

That depends on why it is loose and how much support has been lost.

A tooth loosened by short-term trauma may behave differently from one affected by advanced periodontal bone loss or a root fracture.

Avoid promises that a tooth will “tighten back up” without an examination.

Protect the tooth until it can be checked

Until your appointment:

  • Avoid intentionally wiggling the tooth
  • Chew on the other side if the area is tender
  • Keep the mouth clean gently
  • Do not attempt a home splint
  • Call the office if mobility, pain, or swelling worsens
