Clear Aligner Attachments: What They Do and How to Care for Them
Small tooth-colored attachments may be part of clear-aligner treatment. Learn why they are used, what appointments involve, and when to call your provider.

Clear aligners are often described as smooth, removable trays. If you are starting treatment, you might be surprised when the dentist or orthodontist also mentions small bumps that will be bonded to certain teeth.
Those are commonly called attachments. The American Association of Orthodontists (AAO) describes them as tooth-colored bumps placed on teeth during clear-aligner treatment to help move teeth while the patient wears the trays. They are removed when that part of treatment is finished. AAO: Glossary of Orthodontic Terms
Not everyone needs the same number or placement of attachments. Their design is part of your specific treatment plan.
Why would a clear aligner need attachments?
An aligner applies forces as it seats over the teeth. Depending on the planned movement, small bonded shapes can help the tray engage a tooth and deliver a more specific action.
An attachment is not the same thing as a metal bracket used with traditional braces. It is a small addition to the tooth surface intended to work with a particular series of aligners.
The AAO identifies attachments as one of the components that can be part of getting aligners fitted, and advises regular checkups while treatment progresses. AAO: Clear Aligner FAQs
If you were expecting completely smooth teeth between tray changes, ask your provider to show you where attachments are proposed and what they are intended to accomplish.
Do all patients get the same attachments?
No. The number, shape, and position are based on the teeth and movements being planned. Some treatment stages may not use attachments at all. Your clinician should explain the plan instead of relying on a generic photograph.
A computer simulation can help communicate goals, but it cannot promise that every tooth will move exactly on schedule. The treatment team should monitor how well the trays fit and make adjustments if clinically necessary.

What happens when the attachments are placed?
The clinician prepares the tooth surfaces, uses a bonding process to attach the small shapes, and checks the fit of the aligners. The AAO’s patient FAQ describes cleaning and preparing the enamel, using adhesive and a special light, and confirming that the first aligner seats as planned. AAO: How Aligners Are Applied
You may be shown how to place and remove the tray around the attachments. Ask the team to demonstrate rather than forcing an unfamiliar appliance into place.
The bumps may feel different to your tongue when the tray is out. If you experience persistent discomfort or a sharp-feeling area, tell your treating clinician. Do not try to smooth or remove bonded material yourself.
Can you still brush and floss normally?
Yes, but pay attention to plaque that may collect near the bonded areas. The AAO recommends keeping the teeth clean and brushing after eating before putting aligners back in; food and sweet drinks trapped beneath trays can affect oral health. AAO: Clear Aligner Therapy
Practical habits include:
- Remove the aligners according to your clinician’s instructions for meals and cleaning.
- Brush carefully around attachment edges, and clean between teeth each day.
- Clean the trays using the method your treatment team recommends.
- Keep trays in their case when out of your mouth, not wrapped in a napkin.
- Follow your prescribed wear schedule rather than changing it based on online advice.

What if an attachment comes loose?
If a bonded bump breaks away or disappears, contact your treating dentist or orthodontist for instructions. Explain which tooth appears affected and whether the aligner still fits comfortably.
Do not glue an attachment back yourself or use an old tray to compensate. The best next step depends on which movement was planned and where you are in treatment. Your provider may recommend checking the fit, replacing an attachment, or adjusting the plan.
If the aligner is broken or lost, the AAO similarly recommends contacting the orthodontist for advice rather than improvising. AAO: Clear Aligner Therapy
Will the bumps be removed when treatment ends?
Attachments used for tooth movement are generally removed by the treating clinician when they are no longer needed. Ask how they will be removed and how the enamel will be checked afterward.
Treatment does not stop being important when the final active tray is finished. Retainers are a separate phase intended to help maintain tooth positions; ask your clinician how the retention plan differs from active treatment. The clear-aligner timeline guide covers the broader treatment stages.
Questions worth asking before your next tray
- Which teeth will have attachments and why?
- Will I need attachments for the full treatment or only certain stages?
- How can I tell whether a tray is seating correctly?
- What should I do if a bump breaks or becomes uncomfortable?
- How will cleaning and retainer care change later?
Gateway Dental describes Invisalign and ClearCorrect options for patients considering clear-aligner treatment. A clinician can explain whether attachments would be part of your plan; a photograph or general article cannot determine that for you.
